The National Committee for Quality Assurance (NCQA) announced that Nationwide Children’s Hospital’s Primary Care Network has received Recognition from the Physician Practice Connections –Patient-Centered Medical Home (PPC-PCMH) program for using evidence-based, patient-centered processes that focus on highly coordinated care and long-term participative relationships. The Patient-Centered Medical Home (PCMH) is a promising model of health care delivery that aims to improve the quality and efficiency of care.
